In this article, Protea walks you through 5 core positioning positions so your patient stays safe and recovers as well as possible.

1. Lying on the Back (Supine Position)

This is the most commonly used position, but it carries risks: the hips tend to fall outward (rotate externally) and the affected arm tends to stiffen in a bent position.

Head and neck: Support the head with a pillow so it stays upright, not tilted to either side.

Affected arm: Place a pillow under the shoulder and arm so it sits slightly higher than chest level to reduce swelling. Keep the elbow extended and the palm facing upward. Roll a towel for the patient to grip in the affected hand.

Affected leg: Place a pillow under the hip on the weak side to keep the leg from falling outward. A small pillow under the knee can keep it slightly bent.

Feet: Make sure the blanket doesn’t press the toes down into a pointed position — prop a pillow at the foot end to keep the feet at a right angle.

!Supine position

2. Lying on the Affected Side

Torso: Lie mostly on the side, tilted slightly toward the back, so the shoulder blade on the affected side is drawn forward and the patient isn’t lying painfully on their own shoulder. Place a pillow behind the back so the body doesn’t roll back onto it.

Affected arm: Extend the arm forward at a right angle to the body, palm facing up.

Legs: Position the affected leg behind with the hip extended and the knee slightly bent; bend the strong leg and rest it on a pillow.

Caution: Check that the ear and shoulder aren’t folded under and cutting off circulation.

3. Lying on the Unaffected (Strong) Side

This is usually the position the patient finds most comfortable, but be careful not to let the affected arm get pulled or strained.

Torso and head: Tilt the head slightly forward, keep the body straight, and place a pillow behind the back so the patient doesn’t roll onto their back.

Affected arm: Bring the shoulder forward, and support the whole arm on a pillow in front so it doesn’t drop down onto the bed — a hanging arm can cause shoulder subluxation or shoulder pain.

Affected leg: Bend the knee and hip and rest the leg on a large pillow, parallel to the body. Sitting Position

These same principles apply whether sitting on the bed or in a chair.

Torso: Lean against the wall or headboard, with a pillow supporting the head and back.

Affected arm: Support the weak arm on a pillow with the elbow bent at 90 degrees, to keep the shoulder from slipping out of place.

Sitting on the bed: Stretch both legs out with a pillow under the knees so they bend slightly.

Sitting in a chair: Use a stable chair, with a footrest or with the feet planted firmly on the floor.

5. Technique for Turning a Patient onto Their Side

Arm: Extend the arm on the side you’re turning toward out to the side, and bend the other arm across the body.

Leg: Keep the leg on the turning side straight, and bend the knee on the same side as the arm resting across the body.

The caregiver stands on the side the patient is turning toward, grasps the shoulder and hip on the opposite side — or uses a Protea transfer sheet with grab handles to help pull and turn the patient toward you — then tucks a positioning pillow behind the patient’s back so they don’t roll back flat.

Positions should be changed every 2 hours to distribute pressure. It’s also important to observe the patient’s skin color during each position change: if you find redness that doesn’t fade, avoid lying on that spot for the time being. If a pressure ulcer has already formed, turn and change position as often as possible. If there is no pressure ulcer yet, you can stretch the overnight turning interval to 3–4 hours so caregivers can get more rest.

For stroke patients with hemiparesis (weakness on one side of the body), sleep is not just rest — it is “therapeutic positioning.” If the position isn’t set correctly even for just a few days, it can lead to frozen shoulder, foot drop, or pressure ulcers that are difficult to treat.
